This program provides outreach and essential services for adults, children and adolescents with mental health or substance abuse disorders that have required frequent hospitalization, or pose a risk of hospitalization. Individualized programming offers support for persons who are unable to access and sustain traditional services. Hours, type and intensity of services vary depending on the needs of the individual.
