Mercy Adult Day Health of Westfield is designed to help older adults, as well as adults with disabilities, continue to maintain their lifestyle at home. Caregivers and family members are assured their loved ones are in good hands, receiving the attention and care they require.
Staying Healthy
Mercy’s Adult Day Health program helps enhance and rebuild physical and mental health. As part of our program, the health conditions of each person are closely monitored with appropriate medical support administrated as warranted. Individuals are given training and assistance, enabling them to achieve maximum mobility through the utilization of life skills.
We provide:
- Multidisciplinary assessment of both medical and nonmedical problems
- Risk assessment
- Monitoring and intervention for unstable medical conditions
- Medication management and administration
- Blood glucose monitoring
- Physical, occupational and speech therapies
- On-site lab testing performed by Life Laboratories
- Podiatry services
- Care coordination through social services
- Education programs
- Nutritious meals and snacks
- Personal care assistance
Staying Active
Our social activities all contribute toward an involved, active lifestyle often leading to the development of new friendships. Participants in Mercy Adult Day Health programs share in a wide variety of activities including:

Mercy Adult Day Health is part of the continuum of care provided by the Mercy Continuing Care Network. We welcome adults of all religious affiliations. Mercy Adult Day Health of Westfield is certified by the Massachusetts Division of Medical Assistance. Programs may be covered by Medicaid, Home Care Corporation, the Veterans Administration or privately.
Health care professionals, community agencies, discharge planners and/or family members can initiate referrals to our adult day health program.
