 Licensed social workers provide counseling and support to patients and families and can provide information on a variety of community, public and private services and programs. These might include financial assistance, crisis intervention, mental health services, HIV testing and services, child or adult protective services and substance abuse programs.
Should a patient require admission to an extended care facility, a team of specially trained social workers will assist by arranging for admission and transfer to a sub-acute facility, nursing home, rehabilitation center or rest home.
