|Mercy LIFE is a new health care choice for seniors living in Western Massachusetts. We are a Program of All-inclusive Care for the Elderly (PACE), tightly coordinated care and support designed to help seniors continue to live safely at home and avoid moving into a nursing home.
Mercy LIFE takes the traditional elements of health care—but without the traditional limitations of insurance plans—and uses them in a way that helps you live safely in your own home, even when all of the usual supports available are not enough.
Our interdisciplinary team of doctors, nurses, physical therapists, home health aides, and other health care professionals provide treatment and meet daily to review and report any changes in your health. This results in quick responses to your changing needs, and more flexibility in doing whatever is necessary to keep you safe at home.

|Who is eligible for Mercy LIFE?|
You are eligible for this program if you:
• Are age 55 or older
• Live in Hampden or Hampshire County
• Are certified by the state agency to need a nursing home level of care
• Can live safely at home with our services, as determined by our assessment team
Who pays for Mercy LIFE?
If you qualify for Medicaid or both Medicaid/Medicare, you pay nothing. If you do not qualify for Medicaid, you pay the Medicaid equivalent—a flat monthly fee (a premium)—that will not vary even as your needs increase. Mercy LIFE pays for all authorized care with no additional cost to you.*
|How do I enroll?|
Our enrollment specialist will meet with you (and your family or caregiver) at your home to provide a detailed understanding of the program and begin the process of determining whether Mercy LIFE is the right choice for you. This will include a medical evaluation and development of a care plan. We complete all necessary paperwork so enrollment is easy for you and your family. Enrollment is voluntary—you may disenroll at any time by notifying your Mercy LIFE social worker.
Mercy LIFE Services
Participants must receive all necessary health care services from Mercy LIFE or from an entity authorized by the PACE organization including primary care medical services, medical specialty care, rehabilitative services, nursing care, drug coverage, transportation, medical equipment and supplies, adult day service, social work services, meals (as needed) , personal care, in-home services, outpatient services, inpatient hospital and nursing home care.
* Participants may be fully and personally liable for the costs of unauthorized services or services outside of our PACE program agreement (excluding emergency services).
